Foundation Information for GP Practices (F2)

As part of Modernising Medical Careers new doctors will have an opportunity to do a four month attachment in general practice in the second year (F2) of their two year foundation programme.

The attachments will be in training practices. Supervising doctors can be trainers or educational supervisors who have had some relevant experience such as attending the Foundation Supervisors Course.

If you are currently a Foundation Supervisor and are applying for reacreditation, the appropriate forms are below.  The FY2 Supervisor reaccreditation / appraisal form is the main form to fill in and return to the GP Programme Administrator prior to the first year meeting or reaccreditation panel.  This captures mandatory declarations, updates details of the practice, records supervisor activity, supervisor PDP and reflections of teaching and assessment since the last review.  The form should be completed and returned to the GP Programme Administrator who is coordinating the visit/panel  at least two weeks in advance of the visit/panel.

Foundation Reaccreditation & Appraisal Form 


The trainee feedback on GP Foundation Attachment form is to use to facilitate gathering feedback from your trainees about their experience of the GP attachment.  The hope is that this can be used with each trainee as they come to the end of the attachment to provide the practice with ongoing formative feedback.  These forms can then be included as evidence for the foundation supervisor re-approval visit or reaccreditation panel.  It may provide ideas for ongoing development of these placements.  If the trainee does not want any aspect of the feedback to be shared with the supervisor this should be clearly indicated on the form and emailed directly to the GP Programme Administrator.
